Why coordinators refer to Care to Change
Care to Change is profit-for-purpose and led by people with lived disability experience, so the decision sits with the participant, not a sales target. We are NDIS-registered. Across Support Coordination, Supported Independent Living, Community Participation and daily living supports, we hold a participant’s plan together in one place, from making sense of a plan to the everyday support that follows.
Where we take referrals
We support participants across Greater Brisbane, Logan, the Redlands, Moreton Bay (with hubs at Mount Gravatt and Morayfield) and the Sunshine Coast.
We do not currently cover the Gold Coast, Ipswich, Toowoomba, Gympie, or the northern Sunshine Coast around Noosa, Coolum or Nambour.
We are accepting new referrals across South East Queensland now. Reach out about current SIL availability and intake timing for your participant.

Use the enquiry form on our contact page or call 1300 515 888. Tell us the participant’s goals, the supports they need, and how their plan is managed. We respond quickly and confirm the next steps with you.
Support Coordination, Supported Independent Living, Community Participation and Access, Individualised Living Options, Day Programs and Group Based Activities, daily living supports such as personal care, in-home support and domestic assistance. We are registered for these. We do not offer Specialist Disability Accommodation, respite, or plan management.
We run Supported Independent Living homes across Brisbane, Sunshine Coast, Moreton Bay and the wider region, with a focus on Caboolture and Morayfield. Availability changes week to week, so ask us about current openings before you place a participant. We will not promise a vacancy we do not have, but we may be able to support you in finding a home.
Greater Brisbane, Logan, the Redlands, Moreton Bay and the southern Sunshine Coast. We do not currently cover the Gold Coast, Ipswich, Toowoomba, Gympie or the northern Sunshine Coast.
Our intake team aims to respond within one business day. A fast, human response is what helps you place a participant with confidence.
We are NDIS-registered, profit-for-purpose, and led by people with lived disability experience. Participants get a consistent, multilingual team rather than rotating gig workers, and we do not deduct from their Disability Support Pension.
