Care to Change is a profit-for-purpose organisation founded in 2016. We are a diverse team of compassionate professionals, led by a management group with genuine, first-hand experience in disability care. We understand the challenges families face because we have lived them ourselves, and we use that insight to provide care that is as practical as it is heartfelt.
Based in Brisbane, Moreton Bay, and the Sunshine Coast, we specialise in personalised NDIS support that actually fits your life. From daily assistance and community participation to finding the right accessible housing, we focus on prompt, reliable service. We are here to take the stress out of the NDIS, helping you or your loved ones feel supported, heard, and truly at home in the community.

Frequently asked questions
We provide support coordination (Level 2 and Level 3), supported independent living (SIL), community participation and group activities, including the day program at our Morayfield Hub, and daily living support at home, such as personal care and domestic assistance. Every service has its own page on this site. If you are not sure which support fits your plan, ask us and we will talk it through.
We have two offices you can visit: Mount Gravatt on Brisbane’s south side (Level 2, 1437 Logan Road) and the Morayfield Hub in Moreton Bay (Ground Floor, 19-31 Dickson Road). The Morayfield Hub is also where our day program and group activities run. Support itself is delivered wherever you live, so you do not need to be near an office to work with us.
No. You can contact us at any stage. If you already have a plan, we will talk through what your funding covers and how supports could work for you. If you are still waiting on a plan, or you are not sure what is in it, call anyway. We will answer what we can and point you to the right place for the rest.
It means the money we earn goes back into the organisation, not to outside shareholders. Care to Change has run this way since it was founded in 2016. Surplus is reinvested in our staff, their training and the supports participants receive. That is also why we employ and screen our own team rather than relying on gig-platform fill-ins.
Yes. NDIS participants can change providers at any time. It helps to check the notice period in your current service agreement first, then contact us and we will plan the change with you. We are taking on new participants across our service area, so ask us about current availability.
No. Calling us or sending an enquiry costs nothing, and there is no obligation to go ahead. If you do start supports with us, they are funded through your NDIS plan, and we never deduct from your Disability Support Pension. Call 1300 515 888 or use the form on our contact page.
